What's a Herniated Disc, Pinched Nerve, Bulging Disc...?

There are many terms used to describe spinal disc pathology and associated pain, such as "herniated disc," "pinched nerve," and "bulging disc," and all are used differently and, at times, interchangeably.

Unfortunately, healthcare professionals do not agree on a precise definition of any of these terms, and patients may be frustrated when they hear their diagnosis referred to in different terms. The unusually wide range of terms used to describe spinal disc problems (such as ruptured disc, torn disc, slipped disc, collapsed disc, disc protrusion, disc disease, and black disc) can add to the confusion.

Bulging Disc

You may think a bulging disc is the same as a herniated disc, but there is a difference. With a herniated disc, a crack occurs in the outer layer of the disc, called the annulus. The crack usually affects a small part of the disc, and it allows the soft inner material of the nucleus pulposus to rupture out of the disc. A bulging disc is different because the disc simply bulges outside the space it normally occupies between your vertebrae, but it doesn't rupture. A bulging disc affects a much larger part of the disc than a herniated disc. While it is more common to have a bulging disc than a herniated disc, a herniated disc is more likely to be painful. It's possible for you to have a bulging disc without feeling any pain at all.

Cervical Disc Herniation

I. Description

A herniated disc most commonly occurs in the lumbar spine, but can also occur at the cervical spine level. A disc bulge or protrusion occurs when the annulus fibrosus of the intervertebral disc remains intact, but the increased pressure of the nucleous pulposus protruding from the disk causes compression of any surrounding spinal nerves. A true disk herniation occurs when the annulus fiber ruptures and the nucleous pulposus leaks out from the center of the disc. The nucleous pulposus can cause irritation to the spinal nerves and initiate an inflammatory response. The nucleous pulposus is the gel-like substance of the inner intervertebral disk which primary function is shock absorption. Age is a factor for an increased chance of a herniated disc due to the elasticity and water content of nucleus pulposus decreasing with age.1
